Quiz And Sales Funnel Script

I would like a script to be programmed.
The script, most likely to be done in php/javascript, will allow users to download it and install via FTP to their server and running a install.php or something similar.
Once installed, the user will be able to access a control panel to create their own quiz for the site.
The user can create as many quizzes as they want, with as many answers as they want.
The user can also craft out different responses for their customers who took the quiz. So depending on what the customers answered for the quiz, a different response will be given.
At the end of the quiz, the customer will have to fill up their name and email address so the script can send the appropriate response. Also, the script should allow the user to add in their autoresponder so customer is added to their list.
